Which acne scars respond best

Not all acne scars are the same, and that matters for what to expect. Rolling scars (broad, wave-like dips) tend to respond well. Boxcar scars (wider depressions with defined edges) also tend to respond well. Icepick scars (narrow, deep, and pitted) are the most stubborn and often need a combination approach.

In clinical studies, microneedling has been shown to improve the appearance of atrophic (depressed) acne scars and overall skin texture, with the strongest response in rolling and boxcar scars. It's worth noting microneedling is used to improve the appearance of scars rather than erase them, and it isn't FDA-approved specifically as an acne-scar treatment — a provider can set honest expectations for your skin type and scar pattern. One thing microneedling is not suited for: raised or keloid scars, where it isn't appropriate.
